Eligibility: Resident of area served. Can only be seen once every 30 days.
Application Process: Walk in, call, or visit website for more information.
Required Documentation: Referral, if referred by an agency. For COVID-19 test, ID and Insurance card if available but not necessary.
Fees: No fee for service.
Coverage Note: Monroe County.
