Children's Care Management
thru: Villa of Hope
• Provides Children's Care Management for youth with Medicaid, and Care Coordination services for youth without Medicaid. Supports youth with a mental health diagnosis or history of trauma in meeting their goals and getting connected to the resources they need.
Cultural Competency
Eligibility: Ages 0-21. Must have a mental health diagnosis or history of trauma.

Required Documentation: Call for information.
Fees: No fee for services, Medicaid for Care Management.
Coverage Note: If youth has Medicaid: Monroe, Wayne, Ontario, Livingston, Orleans, Genesee counties If youth doesn’t have Medicaid: Monroe county.
