Hospice Care
thru: Hospice of Central New York and Hospice of the Finger Lakes
• Provides hospice services for individuals in the last phases of a terminal illness.
- Complete range of counseling and medical supportive services.
- Bereavement counseling available for 13 months after the family member's death.
Eligibility: Terminal prognosis in which curative treatments have been exhausted and the physician can certify that the patient has six months or less to live. Resident of area served.
Application Process: Family members, friend or physician can call for information. Hospice brochure is available on website.
Required Documentation: None.
Fees: Coverage through insurance Medicare, Medicaid and private insurance. No one is denied because of inability to pay.
