• Provides case management services for people with intellectual and developmental disabilities.
Eligibility: Age 3 and older. During the intake process, assist with OPWDD eligibility and applying for Medicaid.

Fees: No fee for services.
Coverage Note: Onondaga, Oneida, Herkimer, Madison, Cayuga, Seneca, and Oswego counties.
