thru: New Bethel CME Church
• Provide free health screenings, wellness services, vaccination referrals, and health education services. Health navigators are available to help individuals get connected with community resources.
Eligibility: 18 years and older. Resident of area served.
Application Process: Call for appointment or send a referral through MyWayfinder
Required Documentation: None.
Fees: No fee for services.
Coverage Note: Cayuga, Livingston, Monroe, Ontario, Seneca, and Wayne Counties.
