thru: Greater Harvest Church
• Provide free health screenings, wellness services, vaccination referrals, and health education services. Health navigators are available to help individuals get connected with community resources.
• Hosts free screening events, including Blueprint for Wellness (BFW) and community health fairs.
Eligibility: Resident of area served.
Application Process: Call for information or send a referral through MyWayfinder.
Fees: No fee for services.
Coverage Note: Cayuga, Livingston, Monroe, Ontario, Seneca, and Wayne Counties.
