Eligibility: Adults age 60 years and older, stable income.
Application Process: Call for information intake can be done over the phone or in person, information for visits will be provided over the phone. Referrals are accepted.
Required Documentation: ID, social security card or birth certificate, proof of income. Assistance is provided if needed to obtain any of the documents.
Fees: $800 to $900.
Coverage Note: Monroe County and surrounding areas.
