Children's Single Point of Access - SPOA
thru: Monroe County Office of Mental Health
• Offers Non Medicaid Care Management services, Assertive Community Treatment Services and Mental Health Residential Services.
Eligibility: Must be diagnosed with a mental illness. Community Based services: youth and young adults up to age 21. Residential Services: youth up to 18 years old. Can take referrals for youth and young adults through age 20.
Application Process: Complete application online.
Required Documentation: Call for information.
Fees: No fee for services.
