Children & Family Treatment & Support Services (CFTSS)

Eligibility: Resident of area served. Youth under age 21, enrolled in Medicaid, and have a behavioral health diagnosis.

Application Process: Referrals can be made by anyone familiar with youth and their family. A medical necessity form must be completed by a physician or other medical professional connected to the youth. Referral forms are available on the website or can be requested by email.

Required Documentation: Completed Medical Necessity Form. Insurance card.

Fees: Call for information.
