Service Coordination

thru: Bridges For Brain Injury, Inc

Coordinates all the community services to support individuals to live as independently as possible, including help with: 
  • Transitioning from a nursing home.
  • Adding respite services.
  • Arranging services for someone to live in their own home.

Eligibility: Must be 18 years and older and show proof of brain injury or chronic health/medical issues.

Application Process: Call for an appointment.

Required Documentation: Documentation of Disability from a Physician/Specialist; Medicaid/Managed Care Certification.

Fees: Based on Medicaid/Managed Care Fees Private Pay Offered.

Coverage Note: Ontario, Wayne, Seneca, Yates, Monroe and Livingston Counties.

RTS and Medical Taxi's and Cab Providers transport to our day program from Monroe, Wayne, Livingston, Ontario, Yates and Seneca Counties.
