Organ Donation Program

Facilitates the donation and transplantation of organs and tissue.

Provides public and professional (medical) education on organ donation.

Offers opportunities for residents to enroll in the New York State Donate Life Registry.

Eligibility: People of all ages may be able to pass life on through the gift of organ, eye and tissue donation. Medical conditions at the time of death determine what organs and tissues can be donated. Individuals should not rule themselves out when considering registering as an organ, eye and tissue donor. In New York State, persons 16 years of age or older may enroll in the New York State Donate Life Registry. See "Application Process" below to register to pass life on through the gift of organ donation.

Application Process: You must be at least 16 years of age or older to register as an organ, eye and tissue donor in New York State. Enroll in the New York State Donate Life Registry at www.donatelife.ny.gov There are many ways to enroll in the New York State Donate Life Registry. To register online or mail a printable form, log on to one of the following: • www.donatelife.ny.gov. Click "Enroll" to register online, or click "Forms" to download, print and mail in your enrollment. Forms are available in 7 languages. • www.passlifeon.org (English). Click one of the enrollment options found at the top of the homepage. Other Ways to Register as an Organ, Eye and Tissue Donor: DMV: Check the "organ donation" box when applying for, or renewing your learner permit, driver’s license, vehicle registration, or non-driver license. Voter Registration Form: Check the "organ donation" box on the form. New York State of Health Insurance: Applications for the various health benefits programs provide a section to register as an organ, eye and tissue donor.

Required Documentation: You do NOT need a driver's license to register as an organ, eye and tissue donor.

Fees: There are no costs to your estate or family for you to be an organ donor.

Coverage Note: Cayuga, Chenango, Chemung, Cortland, Jefferson, Lewis, Livingston, Madison, Monroe, Oneida, Onondaga, Ontario, Oswego, Schuyler, Seneca, Steuben, St. Lawrence, Tompkins, Wayne and Yates Counties.
