• Provides in-home support services for adults with complex needs to reside in their own independent apartments.

Go into home to provide services
Go with client into community to provide life-skills
Liaison with other health providers including
- peer bridgers
- community living coaches
- financial case manager
Linkage to community education

Eligibility: Resident of supported housing reinvestment beds (RIV). Ages 18 years or older.

Application Process: RIV: Referral through SPOA. Non RIV: call for information.

Required Documentation: Completed application.

Fees: No fee for service.

Coverage Note: Monroe, Livingston, Wayne, Wyoming, Genesee and Orleans County.
