Eligibility: FLCH Patients on record.
Application Process: By Appointment. Call for Information.
Required Documentation: Insurance card. Proof of income for those requiring assistance.
Fees: Covered by insurance. If no insurance, we offer a sliding fee scale based on income.
Coverage Note: Ontario, Wayne, Seneca, Yates, and surrounding counties.
