Community Health Initiatives

(Program)

* Provides outreach to educate the public, promote healthy behaviors and address the misconceptions surrounding sexual health.

* Prevent the spread of HIV, Hepatitis C and sexually transmitted infections (STIs) in the community through outreach, testing and education. Services included:
  • On-site testing with a mobile van unit
  • Workshops at┬ácommunity locations
  • Special events and support groups
  • Harm reduction services through syringe exchange and overdose prevention training.

Eligibility: None.

Coverage Note: Livingston, Monroe, Ontario, Seneca, Wayne, Orleans, Schuyler, Steuben and Yates Counties.

Languages: Spanish, Sign Language.

Trillium Health is a Federally Qualified Health Center that offers primary medical care (including specialized care for HIV/AIDS, Hepatitis C, and LGBT patients), pharmacy and lab services, behavioral wellness, case management and supportive services, community outreach and education and harm reduction services such as syringe exchange and opioid overdose prevention training.
