thru: Coordinated Care Services Inc.
• Provides one on one assistance to individuals and families who would like to enroll in health insurance through the New York State of Health Marketplace.
• Offers enrollment assistance for those eligible for Medicaid, Child Health Plus, Essential Plans, and Qualified Health Plans.
• Offers enrollment assistance for those eligible for Medicaid, Child Health Plus, Essential Plans, and Qualified Health Plans.
Language Access
Eligibility: Resident of area served and a United States Citizen, National or lawfully present immigrant. Age 65 or younger.
Application Process: Call for an in person appointment. Walk-ins Wed mornings only.
Required Documentation: Call for information.
Fees: No fee for services.
Coverage Note: Livingston and Monroe County.
Languages: Spanish- Staff provides service in language offered, Language line
