• Administers a skilled nursing facility that includes the following services:
- Short-term rehabilitation center.
- Dementia care unit, patients with severe dementia and Alzheimer's disease.
- Geriatric mental health services.
- Medical model day program.
- Transportation arranged or provided to doctors' appointments and day programs.
Eligibility: Must have need for long term care or rehabilitation. Call for information.
