Transitional Care Program
thru: Jewish Home of Rochester
• Provides a short-term rehabilitation facility that offers:
- Care following hospitalization for joint replacement, amputation, motor vehicle accident, stroke or illness that results in deconditioning.
- Physical and occupational therapy provided.
- Home assessment for safety prior to discharge, if needed.
Eligibility: The need for short term rehabilitation and a viable discharge plan. Call for information.

Fees: Medicaid. Medicare. Private pay. Third party insurance. Call for information.
Coverage Note: Cayuga, Livingston, Monroe, Ontario, Seneca and Wayne Counties.
