Care Consultations

thru: Alzheimer's Association, Central New York Chapter

Provides meetings to assist individuals and families to assess current needs and plan for the future in a confidential environment of the interested party’s choosing outside of the home.

Also provides care consultations over the phone 24 hours a day over the National Helpline.

Eligibility: Anyone who has been affected by Alzheimer's disease or another form of dementia.

Application Process: None. Chapter office accepts walk-ins as well as appointments.

Required Documentation: None.

Fees: No fees for service.

Coverage Note: Broome, Cayuga, Chenango, Cortland, Herkimer, Jefferson, Lewis, Madison, Oneida, Onondaga, Oswego, Otsego, St. Lawrence, Tioga, and Tompkins Counties.
