Provides mental health services in the community with a focus on those with serious and persistent mental illness. 

Eligibility: Must reside in Oswego County.

Application Process: Must be referred to Oswego County SPOA through Oswego County Mental Hygiene Division.

Required Documentation: Call for information.

Fees: Insurance including Medicaid accepted.
