Can visit at home if needed.
Helps arrange medical and other treatments.
Assists with finding a place to live, public benefits, and getting other help they may need.

Language Access

Eligibility: Medicaid recipients.

Application Process: Referrals can be completed by family, social supports, providers, or self-referral. Call program manager for assistance completing a referral.

Required Documentation: Medicaid card.

Fees: Medicaid pays for services, no fees for service to clients.

Coverage Note: Monroe County.
