• Coordinates services related to medical, mental health, substance use, and social needs. Works closely with community providers, e.g., schools and mentor programs, to promote success and independence in children and family stability.

Eligibility: Ages 6-21, receiving Medicaid, with a chronic medical diagnosis.
Application Process: Call for referral asking for Care Management Outreach.
Required Documentation: None.
Fees: No fee for service.
Coverage Note: Monroe, Livingston, Ontario, and Wayne Counties.
