Alzheimer's/Dementia Care Program

• Provides individualized, person-directed care for those diagnosed with Alzheimer's disease or other forms of dementia.

Application Process: Call for application, tour. Send PRI, screen, medical and psychiatric history/summary.

Fees: Private pay. Approved Medicaid. Long Term Care Insurance with approval.
Coverage Note: Cayuga, Livingston, Monroe, Ontario, Seneca and Wayne Counties.
