Facilitated Enrollment Services for the Aged, Blind and Disabled
thru: Coordinated Care Services Inc.
• Assists individuals who are aged, blind or disabled with the public health insurance application process for the following:
- Medicaid.
- Public health insurance for individuals with low income. Medicaid can cover nursing home and long term care.
- Medicaid Excess Income/Resource Program.
- A program that provides Medicaid coverage to individuals with income and or resources over the appropriate Medicaid level.
- Medicare Savings Program.
- Assists people on Medicare in paying monthly premiums.
- Medicaid Buy In For The Working Disabled.
- A program for individuals enrolled in Medicare and maybe earning more than the allowable limits for Medicaid.
- Medicare Application Assistance.
- For individuals who are in jeopardy of losing their Medicaid benefits due to not applying for Medicare when eligible.
Language Access
Eligibility: Age 65 or older, blind, disabled, or have active Medicare.
Application Process: Call for appointment.

Fees: No fee for services.
