• Works with Greater Rochester Health Home and Health Homes of Upstate New York to assist clients with getting connected to other resources to improve quality of life.
Eligibility: Must be homeless or at risk of homelessness.
Application Process: Call, walk-in, or visit website. Referrals, including self-referrals, are accepted.
