Bridges To Health Program


* Works with high needs children in foster care who are also at risk for residential placement or hospitalization by improving the health and wellbeing of the youth and family.
* Supports families and social service providers to develop plans and deliver services to address youth needs.
* Strives to support permanency planning.

Eligibility: Up to age 18.

Application Process: Must be referred from DSS. Varies with program.

Fees: Call for information.

Monroe, Livingston, Wayne, Ontario, Steuben, Yates, Chemung, Seneca, Schuyler Counties.
