- Travel.
- Health.
- Adult educational supports.
- Communication, social skills, and leisure skills.
- Money management.
- Positive behavioral support.
- Life safety, hands-on-assistance provided by staff as necessary.
- Professional services as necessary.
- Self-advocacy.
- Informed choice, community inclusion, and relationship building.
Eligibility: Individuals with a developmental disability and who have been approved for eligibility through OPWDD and are HCBS waiver-enrolled. Call for more information.
Application Process: Complete intake application located on website.

Fees: Must be Medicaid-eligible.
