Children's Health Homes

Eligibility: Children from birth to age 21 and must have an active Medicaid AND two Chronic Conditions OR one of the following single Qualifying Conditions: HIV/AIDS, Serious Emotional Disturbance (SED), Complex Trauma.

Application Process: Complete Children’s Health Home of Upstate New York (CHHUNY) referral form. Referrals can be submitted to CHHUNY, or directly to Hillside.

Required Documentation: Completed referral form and proof of eligibility.

Fees: No fee for service.

Coverage Note: Monroe, Ontario, Wayne, Yates, and Seneca Counties.
