Adult Day Health Program


* Provides general health care and social needs of Veteran patients to make it possible for them to continue living at home or with family. Services include:
  • Therapeutic exercise
  • Recreational activities
  • Health education & medication management
  • Meals
  • Care for patients with dementia
  • Support and information for family caregivers

Eligibility: Veteran of one of the US Armed Forces. Honorably discharged. Veterans of allied countries may be treated if care is authorized by said countries.

Application Process: Referral by physician. Veterans may apply in person or by mail-in application. At the time of application, veteran should provide evidence of eligibility as an honorably discharged member of the armed forces by bringing a copy of DD-214 or military discharge papers.

Western New York State including Monroe, Ontario, Wayne, Orleans, Genesee, Wyoming, Seneca and Livingston Counties.
