Nurse Family Partnership
thru: Monroe County Department of Public Health
• Provides perinatal home visiting program for pregnant moms and baby in the home until they are two years of age.
• Assists mothers with having a healthy pregnancy, keeping the child growing and developing normally, learning parenting skills, and achieving goals.
Eligibility: First time mom, less than 28 weeks pregnant, Medicaid eligible.
