Senior Outreach Program

thru: Allegany Rehabilitation Associates**

• Provides non-emergency, in-home mental health assessments to older individuals. Short-term supportive counseling can be provided to address current life issues including depression, transportation needs, grief, confusion, isolation and loneliness, anxiety, and other issues. Case managers are assigned to help individuals make linkages to other programs and services available in the community.

Eligibility: Must be 60 or older or have a disability and be 55 or older.

Application Process: Must have a referral from a doctor, a social service agency, the Office for the Aging, a hospital, a nursing home, the police or other professional.

Required Documentation: Call for information.

Fees: No fee for services.
