Children & Youth with Special Healthcare Needs Program
thru: Wayne County Public Health
• Assists families with referrals to other programs and obtaining resources, such as helping find medical providers, dentists, mental health services, referrals to special practices, and OPWDD Services.
Eligibility: Families with a child from birth to age twenty-one who has a special health care need.
Application Process: Call and ask for the program by name.
Required Documentation: None.
Fees: No fee for services.
