* Provides individual and family support services for older adults that include:
* Provides assessment and treatment for homebound older adults experiencing anxiety and/or depressive symptoms, in home counseling available, up to six sessions and maybe matched with volunteer coach to support independence and connecting with the community through the PATHS program.
- Telephone information and assistance.
- Geriatric wellness screening.
- Case management.
- Benefits advocacy.
- Caregiver training.
- Nutrition centers.

Eligibility: Ages 60 years and older or their caregivers. PATHS/PEARLS: Experiencing anxiety and/or depression. Residing in a community setting (single home or apartment). Unable to access tradtional counseling services.
Application Process: Call for information.
Fees: Sliding scale for some services. PATHS: no fee for services.
