* Provides case management, assistance with Medicaid, SSI, SSD, SNAP, HEAP, section 8 etc. applications, caregiver support, housing transition assistance and culturally sensitive home health care through the Passport Senior Services program.
* Offers a volunteer phone call program for the homebound elderly.
* Provides a national information and referral service for family members seeking to arrange care for elderly relatives living elsewhere through the Elder Support Network.

Eligibility: Client is age 55 and up. Client need not be Jewish.

Fees: Third party insurance as applicable. Sliding scale based on income.
