Provides client centered, individualized service planning and assistance in securing access to services to help individuals in their recovery. These care management services allow caregivers the opportunity to communicate with one another so that all needs are addressed in a comprehensive manner. The Care Management program assists adults, young adults and youth that are experiencing chronic medical, mental health and/or substance use disorders conditions with care coordination and related community-based services. FLACRA’s Care Managers assists clients in coordinating mental health, substance use, medical and social services appointments. They ensure all providers are working together to collaborate and share treatment information to better support the client’s individual wellness and promote self-sufficiency.

Founded in 1973, FLACRA is a private, non-profit agency serving individuals and their loved ones in the Finger Lakes Region whose lives are affected by substance use and other health-related disorders. FLACRA is a Certified Community Behavioral Health Clinic (CCBHC) and a Center of Treatment Innovation (COTI) offering behavioral health care and referral services in 6 counties within the rural Finger Lakes region: Wayne, Ontario, Seneca, Yates, Schuyler and Monroe.
