Eligibility: Pregnant or parenting youth, male or female. Females ages 12-21 years. Males ages 12-21 years.
Application Process: Call for referral form to be filled out and returned or fill out over the phone. Referrals may also be made through Monroe County DHS, Preventive Unit if there is a risk of foster care placement.
Required Documentation: Call for information.
