Comprehensive Health Care and Counseling Services
* Provides primary health care services:
- Adult medicine.
- Specialty care services, including, but not limited to cardiology, endocrinology, sexual health, dermatology, orthopedics, and ophthalmology.
- Physical therapy, chiropractic.
- Laboratory diagnostic testing (samples sent out).
- Women's medicine (Gynecology).
- Physical exams and work physicals.
* Offers outpatient mental health care:
- Individual, family, and group counseling.
- Services provided by volunteer professional health care/mental health providers.
- Medication evaluations.
* Offers health insurance eligibility assessment and enrollment.
* Provides hygiene products on a first-come, first-served basis on Fridays- based upon availability.

Eligibility: Patient/client must not have insurance coverage, including Medicare, Medicaid, or any other insurance plans for services received, or patient must have a significant barrier that prevents them from accessing medical care via their insurance coverage. I.e., Financial debt. Cost-prohibitive copays. High deductible plan. Some exceptions for specialties and chiropractic services. Call for information.

Required Documentation: Proof of income and identification.
Fees: All sliding scale depending on income and household size based on the Federal Poverty Guidelines. New patients: $5-$50. Lab work: $30-$250(cost set by external lab). Visit fee: $5-$50. Payment is expected at the time of the service. Work Physical $85. No insurance accepted. The Health Care Access Department will help to determine fees. Hygiene products: no fee for services.
