* Provides home health aides, licensed nurse practitioners, registered nurses, attendants, and personal care aides to work and oversee medical treatment in the home. Some of the services provided are:

  • Medication reminders
  • Grooming and oral hygiene
  • Bathing and dressing
  • Mobility issues
  • Meal preparation
  • Laundry
  • Housekeeping
  • Errands
  • Companionship

* Provides a full range of services under the TBI/NHTD Waiver that includes the following: Home and Community Support Staff, Community Transitional Services, and Moving Assistance and Transportation. 

* Offers non-medical transportation services.


* Provides care management through Health Homes of Upstate NY (HHUNY) and Children's Health Homes of Upstate NY (CHHUNY).


* Offers tuition free certification for Personal Care and Home Health Aides.

Fees: NHTD and TBI Medicaid Waivers, CDPAP, MLTCs, MCOs, long term care insurance, and private pay. We do not offer sliding scale payments.

Coverage Note: Allegany, Cattaraugus, Chautauqua, Chemung, Erie, Genesee, Livingston, Monroe, Niagara, Ontario, Orleans, Seneca, Schuyler, Steuben, Wayne, Wyoming and Yates Counties.
