- Attend medical appointments and pharmacy visits for medication follow-up.
- Find primary care and other kinds of medical care.
- Coordinate referrals and follow through for mental health and substance abuse.
- Explore other services, like nutrition education, day health, our food pantry, and more.
- Housing assistance and planning.
