* Provides a 24-hour nationwide emergency response service for individuals with Alzheimer’s disease or another dementia who wander or have a medical emergency.
* Covers the cost of the application through a grant from the New York State Department of Health.

Eligibility: Individuals with Alzheimer's disease or another dementia as well as their care partners residing in Central New York.
Application Process: Fill out the application in the Alzheimer's Association, Central New York Chapter's seasonal catalog. Call and a staff member will mail an application. The application can be found on the website in English and Spanish.

Fees: No fee for services.
Coverage Note: Broome, Cayuga, Chenango, Cortland, Herkimer, Jefferson, Lewis, Madison, Oneida, Onondaga, Oswego, Otsego, St. Lawrence, Tioga, and Tompkins Counties.
