* Assists participants to maintain their benefits, access necessary in-home care, attend medical appointments to avoid unnecessary emergency room visits, and feel supported to work on their individual goals.

Eligibility: Must have Medicaid coverage, age 18 or older, medically/clinically qualify according to the Uniform Assessment System and reside in a community setting (or have an interest in relocating from SNF to community setting) in Monroe, Wayne, Seneca, Yates, Ontario, Livingston, and Genesee Counties.

Application Process: Call for information.

Required Documentation: Proof of age (birth certificate/legal ID), proof of Medicaid coverage (insurance card).

Fees: None.
