- Comprehensive Care Management
- Care Coordination and Health Promotion
- Comprehensive Transitional Care
- Individual and Family Support
- Referral to Community and Social Support services & resources

Eligibility: Children from birth to age 21 and must have an active Medicaid AND two Chronic Conditions OR one of the following single Qualifying Conditions: -HIV/AIDS. -Serious Emotional Disturbance (SED). -Complex Trauma.
Application Process: Complete Children’s Health Home of Upstate New York (CHHUNY) referral form. Referrals can be submitted to CHHUNY, or directly to Hillside.
Required Documentation: Completed referral form and proof of eligibility.
Fees: No fee for service.
Coverage Note: Monroe, Ontario, Wayne, Yates, and Seneca Counties.
