* Matches individuals with a care manager to develop a plan to meet medical needs, including:

  • Care management
  • Home Health Aides
  • Medication Management
  • Medical Equipment
  • Physical therapy
  • Speech therapy
  • Respiratory therapy
  • Occupational therapy and more

Eligibility: Individuals 18 and older who are eligible for nursing home level or care, can safely stay in their homes, need long-term care for 120 days or more, are Medicaid eligible, and live in area served.

Application Process: Call or fill out contact form on website.

Required Documentation: Call for information.

Fees: Medicaid.

Coverage Note: New York State except Metropolitan Area Region (Sullivan, Ulster, Dutchess, Orange, Putnam, Rockland, Westchester, New York, Nassau, and Suffolk Counties).
