* Provides comprehensive care management for individuals with chronic health conditions, including mental health needs by coordinating medical, behavioral health, and social services to ensure support and health outcomes. 

Cultural Competency BadgeCultural Competency

Eligibility: 21 years or older, enrolled in Medicaid, resident of area served and have one or more of the following conditions: Significant mental illness or serious emotional disturbance HIV/AIDS Two or more chronic health conditions (i.e. asthma, diabetes, heart disease, mental health condition, substance use disorder, etc.)
