* Offers a 29 bed residential and transitional services that provides rehabilitative services to outpatients who need additional help to prepare for living in a more independent setting and are clinically followed by community mental health centers.
Eligibility: Serious persistent mental illness. Age 18 or older.

Required Documentation: SPOA (Single Point of Access) referral.
Fees: Sliding scale, Medicaid, Medicare. Fee may be waived according to need.
Coverage Note: Monroe, Genesee, Livingston, Ontario, Orleans, Wayne and Wyoming Counties.
