- All requests for assistance via the Prevention & Diversion Program go through an online portal. This portal is ONLY open for requests during the first week of each month.
* Applicants who have illustrated a stable rental history and/or an ability to pay future rent will be supplemented by aftercare case management for up to one year from Family Promise of Greater Rochester staff and/or partner agencies.

Eligibility: Households must have child under the age of 18 or be at least 8 months pregnant to be eligible. Call for information.
Application Process: Visit website - request portal on home page. No applications taken over the phone at this time.
Required Documentation: Applicants must provide either: Rental invoice/balance statement showing the rental amount past due OR copy of lease or landlord statement showing the security deposit required. Applicants must also provide proof of income: Paystubs AND/OR DSS budget sheet or SSI/SSD income statement OR RHA subsidy income sheet. Applicants must sign an Information Release consent form. Please note, security deposit funding is for diversion from shelter ONLY.
Fees: No fees for services.
