Eligibility: Call for information. Trip should start or end in Monroe County.

Application Process: Call at least 24 hours in advance of need. If cars and staff are available, may provide same day accommodations. For trips originating outside of Monroe county please call before sending paperwork.

Required Documentation: Call for information.

Fees: Third Party Insurance. Private pay. Medicaid.

Coverage Note: Primarily Monroe County. No one is turned away.
