* Provides patient-centered care for those who need support with challenges such as depression, anxiety, low self-esteem, poor self-image, trauma, obsessive-compulsive disorder, and anger. 

Eligibility: Individuals from all genders who struggle with mental health disorders ages 12 and up.

Application Process: Call main line for an intake.

Required Documentation: Drivers license/government ID and Insurance card.

Fees: Medicaid, Commercial Insurance, self-pay, sliding scale available.

Coverage Note: Monroe County and counties surrounding Monroe County.
