Eligibility: Any individual with diabetes or other endocrine/metabolic disorders.

Application Process: Call to make an appointment.

Fees: Medicare, Medicaid, and Private Insurance; call for details.

Coverage Note: Cayuga, Chenango, Cortland, Herkimer, Jefferson, Lewis, Madison, Oneida, Onondaga, Oswego, Otsego, Saint Lawrence, Seneca, Tompkins and Wayne Counties.

Cultural Competency: Signage and advertising for diverse populations, Cultural competency, diversity, equity, and inclusion training for staff, Staff reflects population served

Languages: Staff provides in-person interpreter, Language line
