  • Adult educational supports.
  • Communication, social skills, and leisure skills.
  • Money management.
  • Positive behavioral support.
  • Life safety, hands-on-assistance provided by staff as necessary.
  • Professional services as necessary.
  • Self-advocacy.
  • Informed choice, community inclusion, and relationship building.

Eligibility: Individuals with a developmental disability and who have been approved for eligibility through OPWDD and are HCBS waiver-enrolled. Call for more information.

Application Process: Complete intake application located on website.

Required Documentation: See application for requested documents.

Fees: Must be Medicaid-eligible.

Coverage Note: Monroe County.

 

Trinity Assistance Corporation provides supports for people with developmental and intellectual disabilities. The supports provided are all non-certified, community based supports. Services included are Respite, Community Habilitation and Fiscal Intermediary services.
