Eligibility: Income eligibility and participate in Medicaid, SNAP, SSI, TANF, Section 8, HEAP or School Free Lunch program.

Application Process: Call for information, or print application from website and mail or fax in, or apply online.

Required Documentation: Must provide proof of participation in a public assistance program or proof of income. Call or visit website for more information.

Fees: No fee for services.

Coverage Note: New York State.

Languages: Interpreter services available.
